PATIENT INFORMATION
First Time Patient Information:
Preparing for your first appointment:
-
Your first appointment will be approximately 1 hour in duration. We ask that you arrive 15 minutes prior to your first appointment, unless instructed otherwise by Frontier Therapy staff.
-
Dress comfortably for the visit. Please arrive in, or bring comfortable,
non-restricting athletic/workout clothing with you to your appointment. Your attire should also allow your physical therapist to easily access your injured area for treatment.
-
Patients who are minors (under 18 years of age) must be accompanied by a parent or guardian to their first visit.
-
If you need to reschedule or cancel your appointment, please contact us at least 24 hours before your appointment time. We schedule all visits by appointment only, on a first-come-first-serve basis. Cancellations with less than 24 hours notice are difficult to fill. By giving last minute notice or no notice at all, you prevent someone else from being able to schedule in that time slot.
