As a courtesy to our patients, Frontier Therapy will file and assist in obtaining payment for physical therapy services rendered directly with your insurance company.
 

Co-payments are collected at each office visit to avoid you having to remit large co-payment balances. We accept all major credit cards and offer payment installments for those who qualify. Please contact us whenever you have any questions about your account. We are here to help!

 

Before Your Visit: 

Health care plans vary greatly. To assure the highest level of benefit, we encourage patients to be aware of their plan’s coverage. (Your benefits are an agreement between you and your insurance company. Any benefits given by Frontier Therapy, is not a guarantee of payment or actual benefits.) Doctor referrals are not required by law, however, your insurance company may require a doctor's referral.

If you would like to verify your benefits prior to your initial visit, it’s simple!:

• Contact your insurance company via phone

• Provide: Plan & Group Number

• Ask if you need a Doctor Referral for outpatient physical therapy by a liscensed physical therapist to be covered under your plan

  • Yes – if your plan requires a doctor referral then we will need to obtain a referral from your physician. Call your physician and request a referral to Frontier Therapy.
